Payment Policies
Payments:
It is customary to pay for professional services when rendered. We accept major credit cards, checks, and cash for your convenience.

Insurance:
If you have insurance with Medicare Medicaid, and our Doctor is contracted with your carrier, present your insurance card and any required referrals/authorizations to the receptionist at the time of your visit.

Minor Patients:
Nonemergency treatment will be denied for unaccompanied minors unless charges have been pre-authorized to an approved credit card or payment by cash or check. Call the office for more information at (409) 945-551.
Non-Covered Services:
Filing a claim for any service rendered by our office does not mean that your insurance company will pay the charges in full. You will be financially responsible for any services your insurance does not cover.

No Show Policy:
We understand that sometimes you may need to cancel your appointment. To avoid cancellation charges, you need to give our office 24 hours in advance notice, when possible. If you do not cancel your appointment, this will be documented as a “No-Show” appointment, and you will be charged $45. If you do not cancel your surgery appointment 24 hours in advance and do not present to the office for your surgery, this will be documented as a “No-Show” appointment, and you will be charged $200
